You can add extensions to phone numbers on the iPhone. When you are entering the phone number, tap the + * # key and then select the pause button. This will add a space and then a comma to your phone number. Once that's added, you can then enter the extension.

Here's an example of what a phone number with an extension will then look like: (800) 555-1212 ,555
